1. Click on OAG APPLICATION, then download and save a copy of the form as a Word file.
2. E-mail your completed application as a Word attachment to Jim Dahlquist at oag27@mail.state.il.us.
Please type OAG Application in the subject line of your e-mail.
If you wish to
complete your application by hand and mail it:
1. Click on OAG APPLICATION (PDF), then print a copy.
2. Completed applications should be mailed to:
